How to Schedule an Appointment at the CWC
Visit Us
Come to the CWC and ask for an IPAD at the front desk.
Office Location: A-1120
Hours: Mon. - Fri., 8:30 a.m. - 5 p.m.
*Both in-person and virtual appointments are available

Complete the five forms listed below:
- Triage Intake Form
- CCAPS 62
- Consent for Treatment
- Consent & HIPAA
- Availability
Once the forms are fully completed, we will be in touch with you as soon as possible to set up an appointment. Please do not leave any fields blank.
Please note: You MUST BE AN ACTIVE GOVSTATE STUDENT to receive these services. If you are not a GOVSTATE Student, please call the Counseling Lab, which services the community at 708-534-4545.
