Has this course been taught before?
In order to begin the internal review process by the CTL department, the course to be reviewed must have been taught AT LEAST ONCE by the instructor making the request. In order to begin the official review process by Quality Matters staff, the course to be reviewed must have been taught AT LEAST TWICE by the instructor making the request. Master courses cannot be reviewed using this process, because by definition they have not been taught before, but designed as a template to copy to actual running course sections.

Also, if requesting an official QM review, please list all previous semesters after Spring 2020 designed and taught by the same instructor here.
 
Have you completed a Self-Review through MyQM?
A “self review” of the course requested must be completed using QM’s Self-Review tool found at https://www.qmprogram.org/myqm/. If you need help with this process, please contact one of the GSU Quality Matters Coordinators, Nikki LaGrone (nlagrone@govst.edu) or Scott Thesen (sthesen@govst.edu). A GSU Quality Matters Coordinator will follow up with you to obtain your completed Self-Review Report file.
