Students please complete this form if you have been diagnosed with COVID-19. This information will help assess risk to the campus community and deploy appropriate resources.
Reports will only be reviewed during business hours, Monday-Friday, 8:30 a.m. to 5 p.m. If this is an emergency, please call your healthcare provider or 911. Students who test positive should not come to campus and should await further instruction from the Office of the Dean of Students.
