Application for Award

If you have completed all work, and passed all Certificate classes with a B or better, you are ready to apply for a print Certificate in Online Teaching!  

Download the Application for Award of Certificate. This PDF document is editable, so fill in Section 1. If you enrolled in ONTL courses,the College awarding the Certificate is CAS;if you enrolled in NURS courses, then check the CHP option under COLLEGE AWARDING CERTIFICATE.Save the document on your hard drive. Print out the Application and sign it.

You will note that there is place to check that you have attached a copy of the certificate study plan(s).Please fill out the editable Study Plan Form, including the grades you received in each of the Certificate courses, and save it to your computer. Print it out.

Place both completed forms in an envelope and send it to:

The documents will be filed with Registrar; you will receive your print Certificate by mail.
