Directions: My office is located in Room C3359. To reach my office, park in Lot D/West 1 or Lot D/East 1, and head for the front Main Entrance (clearly marked) of the University. Walk through the lobby area (past Student Central) and go to the elevator bank. Take the elevator upstairs to the 3rd floor. Exit right off the elevator, and follow the hallway down and around until you reach the Department of Nursing, where our receptionist will greet you. I'll be happy to see you!

Below are links to all forms required once you are admitted to the MSN, DNP, or PMC-FNP Certificate Nursing program. Please note that the Graduate and Certificate Application Biographical Sketch Form are required only of the graduate and certificate students. All of the other forms (Incident Form, Informed Consent, Potential Disease Exposure Form, Refusal of Hepatitis B Form, Student Handbook Sign-Off Sheet, Universal Precautions Training Acknowledgement, and Student Health Form) are required of graduate, certificate and doctor students, along with a copy of your current RN license, malpractice insurance, personal insurance, and CPR card.

The Grants Office staff of the College of Health and Human Services has created a treasure trove of resources to help you write fantastic proposals, research grant opportunities, access integral data archives, or seek funding for projects. It's called the Research Toolbox, and it's all here in one site:  outlines, templates, data sources, checklists and flow charts.

The Research Toolbox is easy to access and navigate. Just go to MyGSU Portal, and click on My Team Sites, then Other, followed by CHHS Research Toolbox. A handy Research Toolbox User Guide is available below:
